Completion of a bachelor's degree in a health related field. Active or eligible for registration and practice permit with the applicable college.
Additional Required Qualifications:Five (5) years relevant healthcare experience with one (1) year Seniors Health experience. Home Care, Seniors Health, dementia care, discharge planning, and experience with placement or working in DLO environment.
Preferred Qualifications:Knowledge of computer applications (Netcare, Meditech, MS Office); demonstrated competency with RAIHC; AHS McMaster Case Management; Supportive Pathways Training and knowledge of cognitive status and functioning; conflict resolution and negotiation skills; experience with clinical assessment; knowledge of applicable legislation, programs and community resources; and demonstrated ability to navigate the healthcare system and advocate for clients/families.
