Required Qualifications:

Experience with Case Management practice in the community. Baccalaureate degree in a health related discipline. (Graduate level of education preferred.) Current registration with recognized professional body. Current BCLS-HCP registration. Knowledge and experience in working with individuals, families and communities within Adults and Senior’s populations. Minimum of 5 years clinical experience. Community experiences using the Case Management approach an asset. Proven leadership through effective communication, ethical decision making and organizational skills. Monitors and ensures compliance with budgetary guidelines. Excellent communication and facilitation skills within a multidisciplinary team environment. Demonstrated conflict resolution and change agent skills. Basic computer skills. Proven critical thinking and decision making skills. Previous experience in assessing staffing needs, recruiting and hiring appropriate staff.


Additional Qualifications:

This position requires a vehicle for business use. Possession of a valid driver’s license and access to a motor vehicle in good repair for work related travel is required.


Preferred Qualifications:

Minimum of 2 years’ experience in a management or leadership role.
