The Care Manager performs a leadership role in ALA and reports directly to the ALA Clinical Operations Director. The position is responsible and accountable for providing the direction, team leadership and organization over the activities for the Peace River and Fairview Home Care teams in support of the vision, mission and business plan of Assisted Living Alberta. This position is responsible and accountable for making front line unit-level decisions and having direct leadership responsibility for contributing to the achievement of the goals of Assisted Living Alberta. The care manager will be responsible to ensure Continuing Care Health Service Standards are maintained throughout Home Care. Incumbent will also be responsible to ensure that safe patient care is being provided. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

Active registration with applicable Alberta Regulated Health Profession. Three to five years of related experience in clinical practice and previous experience in Seniors’ Health required. Equivalences of education and experience may be considered.

In addition, the incumbent will possess a record of accomplishment in the following areas: Effective communicator with strong organizational skills, influential skills, patient/resident focused orientation and commitment to providing quality services. Leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching and teaching and the ability to inspire and build confidence in others. Demonstrated ability to manage change. Ability to manage human, financial and physical resources within an operating environment. Proven ability to foster partnerships and to achieve organizational goals within an organization and ideally managing within a unionized sector. Strong track record in establishing and maintaining effective working relationships with internal and external stakeholders. Effective leadership abilities combined with a strong commitment to operating within a team environment and the ability to motivate and inspire others to achieve common goals. Excellent analytical and decision-making skills. Ability to foster a climate of cooperation amongst, and build solid relationships with public agencies, government, committees and other partners.
Preferred Qualifications:Minimum 2 years working in a Continuing Care and Homecare setting.
