Your Opportunity:

Many experienced RNs appreciate roles that blend clinical insight with meaningful coordination across the care system, and this position offers that balance. As a Transition Coordinator, you’ll support patients and families as they navigate important care decisions, drawing on information from multiple sources to help them move safely and confidently to the right place of care. The work is fast‑paced, collaborative, and full of problem‑solving - ideal for someone who enjoys thinking on their feet and adapting as situations evolve. Nurses who take a comprehensive view of patient needs and value guiding clients through complex transitions will find this role deeply rewarding. Relocation expense reimbursement may be available for eligible candidates. If this reflects the kind of contribution you’re looking to make, we encourage you to apply. In addition to a competitive rate of pay, Alberta Health Services currently has a Fort McMurray Allowance in place to an annual maximum of $12,480. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

Your in-depth knowledge and skills, advanced judgment and clinical experience, will assist in providing solutions for complex health care issues at all levels with patients, interprofessional teams, administrators and policy makers.
