As an Integrated Home Care Assistant Head Nurse (AHN) for Crowsnest Pass, Pincher Creek, Fort Macleod Home Care and Seniors Health Education - South Zone, you will be working in close partnership with the Integrated Home Care Manager to support a compliment of busy and progressive Integrated Home Care Teams/Programs to ensure that safe, timely, comprehensive and quality coordinated care is provided. This position will be based out of the Crowsnest Pass (Blairmore) Home Care office with expectations of travel to other sites within the assigned portfolio required. This position will work closely with the various health care providers to serve clients receiving care within any of our South Zone Continuing Care programs (Palliative Care, Home Care, Supportive Living, and Long-Term Care). Reporting to the Area Manager for Crowsnest Pass, Pincher Creek, Fort Macleod Home Care and Seniors Health Education - SZ the AHN is responsible and accountable for front-line clinical support, acting as a resource and in a consultative capacity while leading a team of multidisciplinary health care providers within the Integrated Home Care Program. The AHN is actively involved in resource allocation, patient flow, implementing and maintaining clinical best practice standards, and workflow optimization. The AHN position demands demonstrated ability in change management, strategic and collaborative leadership, and ability to engage a multidisciplinary team in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
Description:
As an Assistant Head Nurse, you will be responsible for staff assignments, patient flow and acting as a role model, clinical resource and advocate for nursing staff. In addition to providing frontline nursing care, as required, you will take a leadership role in the promotion of quality patient care and the implementation of operational priorities. Your in-depth knowledge and skills, advanced judgment and clinical experience, will assist in providing solutions for complex health care issues at all levels with patients, interprofessional teams, administrators and policy makers. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Minimum 3-5 years of experience in home care with preference given for system experience (acute, palliative care, transition services) Proven leadership and communication skills, including demonstrated ability and comfort with decision making responsibilities, coaching and teaching, and the ability to inspire and build confidence in others Excellent organizational, prioritization, and change management skills Ability to perform all assigned tasks and direct clinical care including but not limited to: constant adjustment of position (sit/stand) throughout the shift, potential for back strain from prolonged sitting, keyboarding/mousing documentations for prolonged periods of time. -Ability to travel between assigned areas as needed -Demonstrated clear pattern of professional and personal development -Ability to maintain effective and respectful working relationships -Ability to follow and adhere to AHS policies, procedures, goals and objectives -Progressive leadership experience in a Continuing Care environment with demonstrated ability coaching for performance, mentorship and conflict management, and management of diverse human and material resources in a changing and complex environment -Ability to positively influence an environment of workflow improvement, patient safety, best practice, and continuous quality improvement -Ability to foster partnerships and strong relationships with internal AHS programs and services, and external agencies
Preferred Qualifications:Baccalaureate Degree in nursing completed or in progress with intent to complete preferred however, skills and experience relevant to nursing practice in the community sector will be considered, Intravenous Management Certification, CVAD/IVAD Certification, PICC Occlusion Management Certification AHS Nursing Competencies Case Management Certification interRAI Certified CCHSS Audit Experience
