Required Qualifications:

Completion of master's degree in Social Work. (equivalent education or experience may be considered). Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Heart and Stroke Foundation BLS Health Care Provider. Experience working in outpatient setting as a medical social worker, within the last 5 years. Advanced level clinical intervention and assessment skills in crisis intervention/family therapy, psychotherapy/counselling with patients & families facing chronic illness. In-depth knowledge and experience in working with Income Security programs at community/provincial /federal levels (Alberta Works, AISH, CPP). Experience with interventions that align with legislative acts (e.g. PDA, AGTA, CYFEA, MHA, PPCA, and HIA), housing, Decision Making Capacity and Alternate Decision Making processes and options (Guardianship, Trusteeship, Personal Directives, Enduring Power of Attorney, Specific Decision Maker, and Co-decision), community resources, financial resources, end of life and bereavement care, domestic violence, addictions and counselling are required for this role. Ability to establish positive working relationships with a wide variety of patients and families. Demonstrated ability to work effectively within a patient-centered, interdisciplinary team of health care professionals. Excellent communication skills, written and verbal, and prioritization skills. Demonstrated ongoing professional development education and dedication to personal and professional self-development through a variety of learning mediums.

Preferred Qualifications:

Proficiency with SCM electronic charting and the Millennium scheduling system. Experience working with an outpatient neurological population. Demonstrated leadership in education, group facilitation and clinical practice while working within a multidisciplinary team.
