Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. A minimum of 5 years working as a Social Worker. Recent, within the last 2 years, clinical experience in chronic disease management with adult populations. Recent clinical experience, within last 2 years, working in a community/outpatient setting. At least one year experience working within the acute care setting in the last 5 years. In-depth knowledge and experience in working with Income Security programs at community/provincial /federal levels (Alberta Works, AISH, CPP). Knowledge of and experience with interventions that align with legislative acts (e.g. PDA, AGTA, CYFEA, MHA, PPCA, & HIA), housing, Decision Making Capacity & Alternate Decision Making processes & options (Guardianship, Trusteeship, Personal Directives, Enduring Power of Attorney, Specific Decision Maker, and Co-decision). Experience with and knowledge of community resources, financial resources, end of life & bereavement care, addictions & counselling.

Preferred Qualifications:

Experience working with chronic disease management with adult populations in an outpatient setting within the last year. Experience in taking a key role in program or service development/change. Ability to provide clinical service within a continually changing health care environment. Demonstrated dedication to personal & professional self-development through a variety of learning mediums on a yearly basis.
