Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

A minimum of 5-8 years’ experience in adults in Mental Health, Home Care, Geriatrics or Transition Services. Equivalencies with either adult medical/surgical clinical background or community experience (SL and LTC) will be considered. Strong level of computer skills as this role requires the use of a laptop computer and mobile phone. Physically able to manage (30 lbs.) lifting, pulling, pushing and up to 5 km walking as part of the role. Excellent organizational, oral and written communication skills; high level of independence and decision-making ability.

Preferred Qualifications:

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice will be considered. Previous community and/or geriatric or mental health experience. Previous experience working with Stakeholders and multidisciplinary teams. Understands and is able to identify standards and philosophical principles for Case Management. Experience with RAI-HC, PARIS, Strata Health pathways and Sunrise Clinical Manager Electronic Health record.
