Your Opportunity:

The Emergency Department PCN Liaison at the WestView Health Centre reports to the Program Manager of Ambulatory and Transition Services and has accountabilities to the WestView PCN. The Registered Nurse - Clinical Liaison, Emergency Room works to the full scope of practice of the Registered Nurse and is responsible to access, plan, implement and evaluate interventions provided to identify patients either currently in the Emergency Department or when indicated, those discharged from the site. The Clinical Liaison support the Emergency Department to transition care from acute to primary care. The Clinical Liaison will assist in facilitating primary care follow-up by the PCN with the patient after an Emergency Room visit. A key function of the role is to improve care for complex high needs patients seen in the Emergency Department. This will include active case finding and exploring creative ways to support quality care by utilizing case management principals. The Clinical Liaison is required to work independently, but also works collaboratively with the health care team members both internal and external to the site in order to support seamless, quality health services to patients. The Clinical Liaison is required to consistently demonstrate high quality communication skills and strong problem solving abilities. The Clinical Liaison requires a working knowledge of Emergency room operations, and community resources internal and external to AHS.


As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Three years of recent Emergency / Acute Nursing experience required. Previous experience in Home Care or related setting required. Demonstration of quality assessment, critical thinking, judgment and problem solving abilities. Excellent interpersonal communication and organizational skill required. Able to work effectively both independently and in a team environment.

Preferred Qualifications:

Degree in nursing preferred. Strong knowledge and / or advanced education related to microbiology and diagnostic reporting and associated physician orders is an asset.
