Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Strong verbal and written communication skills; ability to engage in meaningful and solution-focused discussions with stakeholders and partners is imperative. The ability to organize one’s self and remain organized during times of priority shifting and pressure is imperative in Case Management; there are ongoing multiple, competing priorities that shift daily. Strong computer and technology skills (or ability to quickly learn and adapt) is required due to electronic documentation and care planning requirements for SL. Strong negotiation and mitigation skills; ability to remain professional and composed during times of adversity. Ability to conduct (clinically competent) and experience with comprehensive physical assessments. Ability to problem solve and communicate abnormal findings within appropriate time frames based on a risk assessment. Experience with care plan development and complex case conferencing to overcome situations.

Preferred Qualifications:

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in the community sector will be considered. Recent community related nursing experience (obtained within the last 5 years) is an asset. Previous experience working with clients who are geriatric, have mental health diagnoses, and with clients who have complex medical health needs (e.g., COPD, end of life status, diabetes) is an asset.
