Required Qualifications:

Completed a bachelor’s degree in a health care profession, health management or related field. Active or eligible for registration and practice permit with the applicable college.


Additional Required Qualifications:

Three to five years’ Senior Health experience that includes home care and transition services.


Preferred Qualifications:

Experience with home care & transition services; Knowledge of computer applications (MS Office) & Meditech; Experience with RAIHC & Designated Living Option Guidelines; Experience working with interdisciplinary teams; Strong leadership abilities; Effective communicator with negotiation & conflict resolution skills; Knowledge of applicable legislation, programs/community resources; Ability to establish team goals & achieve outcomes; Ability to multitask and prioritize workload.
