Required Qualifications:

Completion of bachelor's degree in Health Promotion, Health Sciences, Health Education or a related field.

Additional Required Qualifications:

Current registration with appropriate Provincial/National Professional Associations as required by legislation. Current, valid Class 5 driver’s license. Current CPR (BCLS) – Level C. Lay Leader Certification in the Stanford Chronic Disease Self-Management Program. Formal education or certification in Adult Education. Evidence of ongoing professional development. Experience in self-management support, patient education, behavior change, and adult education is required. Minimum of three years’ experience in community or primary care and/or experience with self-management support, chronic disease management and prevention.

Preferred Qualifications:

Stanford T-Trainer or Master Trainer certification preferred. Additional training in patient-centered care, self-management support and behaviour change principles, and the Expanded Chronic Care model are considered assets.
