Your Opportunity:

Reporting to the South Zone Integrated Quality Management team manager, the CoACT Team Lead will provide guidance and leadership in the planning, implementation and sustaining of collaborative care in acute care and long-term care within South Zone AHS. The incumbent will work closely with program leadership, physicians, frontline staff, patients and families to facilitate a culture of patient and family centered care and to promote a culture of inquiry, curiosity, quality and safety. Responsibilities include but are not limited to: working closely with point of care teams, providing education, customizing resource materials, supporting change management activities, facilitating quality improvement, collaborating with site, Zone and provincial Collaborative Care leads, fostering ongoing learning and partnerships, and reflection and modeling of professional collaborative practice. All Non-Union Exempt positions are being reviewed for possible inclusion in the AUPE General Support Services bargaining unit. If this position is impacted, the incumbent will be notified accordingly.


As the CoACT Team Lead you will support a wide variety of nursing roles to align with leading collaborative care practices in care delivery, while taking necessary steps to ensure patient safety and well-being. In your role, you will use collaborative processes to integrate leading practices into care through critical thinking, problem solving, and decision making. As well, you will teach, counsel and advocate for patients and families. You will play a leadership role in supporting an integrated and holistic approach to patient care health promotion and maintenance. You will support and provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

A minimum of a Baccalaureate degree in a nursing or allied health field with current registration and membership in good standing with a professional regulatory college as required. Minimum 5 years clinical experience in a variety of clinical settings, including some experience in acute care. Adult education and prior staff development experience essential. Demonstrated understanding of change management concepts. Strong interpersonal/communication skills. Demonstrated leadership skills and abilities. Demonstrated strength working with interdisciplinary teams and a variety of stakeholders at a provincial level. Strong computer skills. Demonstrated commitment to AHS vision and values. Ability to mentor adults in a collaborative supportive manner that allows them to model, observe and provide constructive feedback to colleagues. Demonstrated working knowledge of Collaborative Care, Patient and Family Centered Care, and Quality Culture. Flexible, creative and able to adapt easily to a changing environment.

Preferred Qualifications:

Master’s degree in a health-related area and/or education. Previous experience as a CoACT Lead and completed CoACT Leadership Certificate preferred. Planning or project management experience would be an asset.
