Required Qualifications:

Completion of a bachelor's degree in a health related field. Active or eligible for registration and practice permit with the applicable college.

Additional Required Qualifications:

Five (5) years relevant healthcare experience with one (1) year Seniors Health experience. Home Care, Seniors Health, dementia care, discharge planning and experience with placement or working in DLO environment.

Preferred Qualifications:

Experience with Home Care, Acute Care, & transition services; Knowledge of computer applications (Netcare, Meditech, MS Office); Experience with RAIHC;AHS McMaster Case Management; Experience working with interdisciplinary teams; Ability to advocate for clients/families; Experience navigating the healthcare system/community resources; Negotiation & conflict resolution skills; Experience with clinical assessment; Knowledge of applicable legislation, programs and community resources.
